Re-Enrollment Form

  • If you are a NEW family, you will click on the “New Parent Registration” link underneath the login and complete the form to enroll you child(ren).
  • If you are an EXISTING family, simply enter your current Orbund username and password and proceed to re-enroll your child(ren). Please note, much of your previous information will be saved on Orbund, but all families will still be required to submit updated medical information and re-submit the economic deprivation survey for the upcoming school year. In addition, please update any other records, i.e. emergency contact information, as needed.

As you fill in the online form, remember that items marked with a red asterisk and labeled as “required” must be filled in order to move on.

Your enrollment/re-enrollment for the 2017-2018 school year is complete when you have:

  1. Completed and submitted the online application
  2. Submitted your registration fees and documents to the ADS front office (registration fees and documents are listed below for new and existing families).  

It is important that parents complete both items above in order to enroll/re-enroll for 2017-2018, as classroom rosters are filled on a first-come, first-served basis. Priority will go towards families who have submitted all documents and fees. Students whose documents and registration fees aren’t submitted will be placed on a waiting list. 

 Registration for EXISTING Families

Once you submit your online re-enrollment form, please bring the following documents and fees to complete registration process for 2016-2017:

  1. Re-enrollment Fees:
    Application ($50 by 4/30/16, $100 after 4/30/16), books ($300), & technology fee ($50 for grades 1-12 only):

     -$400 per student in grades 1-12
     -$350 per student in PreK-3, PreK-4, & Kindergarten
  2. Updated immunization records
  3. Photo IDs of 2 Emergency Contacts listed on registration form

Registration for NEW Families

In addition to submitting an online enrollment form, you will need to bring the following documents with you to complete the registration process for the 2017-2018:

  1. Enrollment Fees:
    Application (Fee is waived until 3/10/17, $50 by 4/30/17, $100 after 4/30/17), books ($300), & technology fee ($50 for grades 1-12 only):
     $400 per student in grades 1-12
     $350 per student in PreK-3/4, & Kindergarten
  2. Birth certificate
  3. Updated Immunization Records
  4. Photo ID for parents/guardians
  5. Photo ID of emergency contacts listed on registration form
  6. Transcripts from most recent school for students entering grades 1-12. (Most recent report card will suffice until transcripts are available)
  7. Assessment Test (for students entering grades 1-8)
  8. Student Essays (grades 6-12) submitted to admissionsteam@alsalamdayschool.org or ADS front office
  9. Student Interview with Admissions Committee (grades 6-12): Dates to be determined. Student Essays must be submitted at least 3 days prior to any scheduled interview with admissions committee.

Registration requirements for all students

  1. Birth Certificate
  2. Social Security Card
  3. Current Immunization Records
  4. Proof of Legal Custody (when applicable)
  5. Records Request Form (new students only)
  6. Completed Application

Criteria for Admission to Preschool

  1. Child must be 3 years old by September 1st.  A copy of his/her birth certificate must be provided as proof.
  2. Child must be toilet trained.
  3. A copy of the child’s immunization records must be provided as proof

Criteria for Admission to Kindergarten

  1. Student must be 5 years old by September 1st.  Copy of birth certificate is required.
  2. A copy of an up-to-date Immunization Record Card must be provided.
  3. A signed copy of a complete physical exam done by a physician must be provided.

All other students must provide a copy of their birth certificates and up-to-date immunization records.
